• You can only use letters (A-Z), numbers (0-9), periods (.), apostrophes ('), dashes (-), or blanks (spaces). No other characters are allowed.

• If your mother or stepmother does not have a last name, enter her first name. Use her proper name, not a nickname.

Your mother's or stepmother's Social Security Number (SSN) and last name must match exactly the number and name on her Social Security card. To update the information on a Social Security card, call the Social Security Administration at 1-800-772-1213 or go to www.ssa.gov.
